Our client in Atwater, CA is looking for a reliable, passionate Physical Therapist with Home Health experience to provide care to their patients in the Atwater/Merced, CA area.

This position offers flexibility to create your schedule, 40 hours per week. We offer local pay or travel pay if you live farther than 50 miles from the home base office in Atwater, CA.

JOB DUTIES:

  • Obtains all pertinent medical history from patient, family, or significant others

  • Performs PT assessment visit and documents accordingly

  • Performs home safety check and environmental assessment of the patient’s home environment

  • Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation

  • Functionally reassess the patient for on-going therapy

  • Determines medical necessity for other services

  • Evaluates the patient’s ADL and iADL abilities

  • Effectively communicates ongoing with patient and family regarding the POC and progression of such

  • Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc)

  • Effectively communicates with other disciplines in the case

  • Communication with the patient’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities

  • Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests

  • Submits ALL OASIS assessments/documentation within 48 hours of the OASIS visit

  • Participates in staff meetings and in-services

  • Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care physical therapy and the benefit of the patients serviced

  • Other duties as assigned

REQUIREMENTS:

  • Current CA Physical Therapist License

  • Current BLS

  • 2 year min PT experience. Or 1 year Home Health experience.

  • Reliable transportation (Mileage reimbursed)

POSITION DETAILS:

  • Location: Atwater/Merced

  • Length of Assignment: 13 weeks with opportunity for extension

  • Schedule: Mon-Fri. Appointments are between 8a-5pm. Weekend flexibility if needed.

  • Other details: Must have reliable transportation. Mileage reimbursement will be paid at $.67 per mile. Travel between homes is about 100 miles per week.

  • Pay

  • Travel (must live more than 50 miles from facility): $51/hour + $749 Lodging stipend weekly + $413 Meal stipend weekly = $3,202 per week

  • Local (must live within 50 miles from facility): $67 per hour = $2,680 per week
