LPN/LVN --- HOME HEALTH.
Private Duty Nursing - Ratio 1:1. One home per shift; Various Shifts - facility is open to 5X8; 4X10; 3X12 - ensure your candidate advises what shifts they would like
Patients may be on trach, g-tube, vents,
Pediatric experience is highly preferred
Start date: must start within 3-4 weeks of submittal
Ratio: 1:1
Years of experience REQ: 1 year experience
Weekend REQ: EOW
Certs REQ: BLS
Is on-call REQ? No
Will clinician float within scope to meet facility needs? No
Open to accommodating block schedule? No guarantee but will try if it fits the facilities needs
Locals accepted: YES
Pending License accepted: Yes – must show proof of license pending and advise on date applied – depending on date applied, candidate may be redirected
RTO Restrictions: 1 week max – must make up if it’s a whole week
Guaranteed Hours: 36

The LPN / LVN Pediatric Nurse functions under thesupervision and guidance of the RN/Nursing Supervisor or Physician andparticipates in the multi-disciplinary process of providing care and treatmentto patients and provides clinical support and technical assistance to theprofessional medical and other staff. The LPN is responsible for functioning within the scope of practicedictated by the respective Board of Nursing.
Essential Duties and Responsibilities:
Takes and records vital signs of pediatric patients
Prepares and gives injections and enemas; monitors catheters
Assists patients with bathing, dressing, and other personalhygiene
Assesses signs and symptoms indicating physiologic andpsychosocial changes in the patient’s condition
Assists physicians with patient office visits: includingmanaging the front desk with phones, scheduling appointments, patientreferrals, putting patients in rooms, interviewing patients, recording vitalsigns including pulse rate, blood pressure, height and weight, and entersinformation in patient’s electronic medical record
Performs interventions according to priorities, plan ofcare, and the hospital/clinic policies and patient care outcome standard
Provides a safe, comfortable and therapeutic patientenvironment
Uses clinical judgment in evaluation activities to meetpatient care needs of an assigned unit/floor including establishing priorities
Monitors the safety and effectiveness of the environment andequipment
Administers immunizations
Has strong knowledge of pediatric pharmacology
Minimum Requirements:
Current LPN/LVN nursing license within the state ofassignment
Minimum of one year pediatric clinical experience within thelast three years preferred
Current PALS Card (per client and / or state requirement)
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors, co-workers,clients) via strong communication skills; proficiency in the English languageis required
Computer proficiency required
Must be at least 18 years of age
